The Turczyniak Scholarship
Oakville Hornets Girls Hockey Association
The Oakville Hornets Girls Hockey Association is proud to offer The Turczyniak Scholarship, consisting of two (2) $1,000 scholarships awarded annually to graduating Grade 12 players who have demonstrated growth, leadership, commitment, and character through their involvement in the Hornets community.
This scholarship was established by Mike Turczyniak, past President of the Oakville Hornets Girls Hockey Association. Mike created this scholarship to recognize and celebrate devoted graduating players who have spent years within the Hornets organization, consistently showing commitment, dedication, teamwork, and heart—both on and off the ice.
One scholarship will be awarded to a Grade 12 Rep Program player, and one scholarship will be awarded to a Grade 12 House League Program player. Each scholarship is intended to support the recipient’s post-secondary education at a recognized college or university.
The Turczyniak Scholarship is designed to recognize more than athletic performance. Its purpose is to honour players whose experiences within the Oakville Hornets have contributed to their personal growth, leadership development, and positive involvement within their team and the broader community. This scholarship celebrates the lasting impact hockey can have in shaping confident, responsible, and compassionate young women.
All applications must be submitted by March 3, 2026.
Application Process
Applicants must complete two components:
1. Application Form
Applicants are required to complete the online application form provided by the Oakville Hornets Girls Hockey Association. Turczyniak Scholarship Application – Fill out form
2. Video Submission
Applicants must submit a 3–5-minute video recording, filmed using a phone or similar personal device.
The video should be a personal reflection responding to prompts from five categories listed below.
Applicants may answer one or both prompts within each category.

1. What is something hockey taught you about yourself that you didn’t expect?
2. What has being a Hornet meant to you?
Team & Community
1. How would your teammates and/or coaches describe you?
2. Describe a time you supported a teammate off the ice.
Leadership & Responsibility
1. What does leadership look like to you, even if you were not a captain?
2. How has being part of the Oakville Hornets shaped who you are today?
Life Beyond Hockey
1. What skills from hockey will help you succeed in college or university?
2. Describe how lessons from hockey have helped you overcome a challenge outside of sport.
Looking Back & Looking Forward
1. What advice would you give to a younger Hornets player?
2. How do you see yourself giving back in the future, whether to hockey or your community?
Simple, unedited recordings are preferred. Professional editing, special effects, or production quality are not required or expected. Authenticity and reflection are valued most.
Video Submission Guide
As part of your application for The Turczyniak Scholarship, you are required to submit a short video reflection.
Step 1: Record Your Video
Use your phone or another personal device. Record a 3–5-minute video. You may speak naturally—notes are allowed, but please do not read directly from a script. There is no need for editing, filters, music, or special effects. A simple, unedited video is preferred.
*Be sure to answer at least one prompt from the five categories.
Step 2: Upload Your Video
Once recorded, upload your video to Google Drive or OneDrive. Ensure the video is fully uploaded and plays properly. Set the sharing permissions to “Anyone with the link can view.” Do not restrict access or require sign-in approval.
Step 3: Submit the Link
Copy the shareable link to your video and paste it into the designated section of the Microsoft Form (Turczyniak Scholarship Application – Fill out form)
*Double-check that the link works before submitting your application.
Important Notes
Videos must be 3–5 minutes in length. Late or inaccessible video links may result in the application being considered incomplete. Video quality does not affect evaluation—authenticity matters most.
